RELEASE OF LIABILITY, WAIVER OF CLAIMS AND ASSUMPTION OF RISK - Champion Sports Academy 1. Assumption of Risk: I understand that tennis, badminton, pickleball and related training involve inherent risks, including falls, collisions, being struck by balls or racquets, muscle and joint injuries, dehydration, and other injuries that may be serious. I freely accept these risks on behalf of my child. 2. Rules and Conduct: My child will follow the instructions of coaches and staff and the facility rules. The academy may remove a participant for unsafe or inappropriate behaviour. 3. Health: My child is physically fit to participate and I have disclosed any medical conditions, allergies or injuries. I will update the academy if these change. 4. Medical Treatment: In an emergency, I authorize the academy to obtain first aid and emergency medical treatment for my child, and I accept responsibility for any related costs. 5. Release and Waiver: To the fullest extent permitted by law, I release and waive any claims against Champion Sports Academy, its owners, coaches, staff and volunteers for injury, loss or damage arising from my child's participation, including claims for negligence. 6. Indemnity: I agree to indemnify the academy for claims, costs or damages caused by my child's or my own actions. 7. Personal Property: The academy is not responsible for lost, stolen or damaged personal items. 8. Governing Law: This agreement is governed by the laws of the Province of Ontario. If any part is found unenforceable, the rest remains in effect. 9. Acknowledgment: I have read and understood this document, I am the parent or legal guardian of the participant, and I sign it voluntarily.
